how to define custom paper ?

How do I assign a custom paper to a Generic / Text only printer ?
In Windows 98 there was an option called "Custom" at the end of the list of
paper sizes where I could specify the width and the length.
I can't find that option in Windows XP.

Any help will be greatly appreciated.
