Why must I click Restart and Shutdown twice?

OS: Win XP SP2 with latest updates installed.

Around the time I installed SP2 my computer now requires two clicks to
restart or shutdown. A number of items drop off the task manager processes
list but computer keeps running.

What can I do to resolve this?

I had this same problem.

Are you using Quickbooks?

I found that if I exit "Quickbooks Database Server Manager" via the icon in the taskbar (by the clock) that I can then shutdown or restart normally.

I don't know why Quickbooks causes this to happen but exiting it solved the problem for me...

If it is not Quickbooks, I recommend looking at any other programs that are running at the time you try to shutdown.
